Graduate Forms and Guidelines

Petition to Graduate Form 2013 and January 2014 is for those planning to graduate. Please complete this form and return it to the Graduate School office.
Degree Candidacy Forms is required upon the completion of 12 credits in a degree program.

Teaching Excellence Award Nomination Form
To nominate a professor for the Graduate School Teaching Excellence Award, please submit the Teaching Excellence Nomination Form. Nomination forms must be returned to the Graduate School office. The recipient of the Graduate School Teaching Excellence Award will be announced at the Graduate School Reception for Master's Degree Recipients immediately following commencement in May.
Transcript Request Form is for applicants to the Graduate School to submit to their previous institutions.
Notification of Intent to take the Comprehensive Examination 2013 Required for students in the Master of Arts in Thanatology and Human Sciences programs.
Notification of Intent to take the Biomedical Science Comprehensive Examination 2013 is required for students pursuing the non-thesis track.
Graduate Research Funds Guidelines and Forms

Permission to Enroll form is for registration in independent studies, practicums, theses, projects, capstones and directed readings. A brief written proposal must also accompany this form.
Incomplete Grades form must be completed and signed by the instructor and student should extenuating circumstances prevent a student from completing a course within a given semester.
