Accommodations Request Process & Guidelines for Documentation

All Hood College students with documented disabilities who wish to request accommodations must provide required documentation and meet with the Office of Accessibility Services.

Below are links to the Guidelines for Documentation and Disability Verification forms. Review the Guidelines for guidance on to required documentation for specific disabilities.

To ensure Hood college is able to meet the needs of students with disabilities in a timely manner, documentation and diagnosis of a specific disability completed by a qualified examiner should be mailed, emailed or faxed to the Office of Accessibility Services as soon as possible.  If submitting information for a new academic year, we request that you submit your requests by June 15 preceding the fall semester.

After your request is submitted by email or once documentation has been received, the Office of Accessibility Services will contact you to make an appointment to discuss your requested accommodations.  

If your request for accommodations is denied, you may appeal the decision.

Confidentiality Statement

Disclosures to OAS are confidential and will never appear on a student's college record. OAS does not share information regarding eligibility for accommodations with other college staff or faculty members unless authorized in writing by the student. Students are welcome to disclose a disability and receive guidance without needing to request or use any accommodations.
