Hood to Launch Health Hubs Advancing Community Health Education in Frederick County
Hood Health Hubs, hosted by Frederick County Public Libraries, to advance population health practices in community settings

FREDERICK, Md., — Hood College and the Frederick County Health Department (FCHD) are proud to launch Hood Health Hubs, a new initiative that aims to expand clinical practice in community health settings and improve population health through education.
The Health Hubs will be hosted by Frederick County Public Libraries (FCPL) in at least two of the nine public libraries in Frederick County during their first year. They are supported by a five-year implementation grant from the Maryland Higher Education Commission’s Nurse Support Program II.
Led by Hood faculty and nursing students, Hood Health Hubs will provide community-based access points for individuals to share health concerns so that risks and barriers can be identified, prevention and early detection of chronic disease can be prioritized, and population health action plan measures may be met.
“I am grateful to the Maryland Higher Education Commission and Nurse Support II program for their support of a new population health initiative this year,” said Jennifer Cooper, DNP, chair of the Department of Nursing at Hood College. “Hood Health Hubs will enrich student learning in community health, and we are excited to work with our community partners to help meet needs in the community.”
The partnership will provide Hood’s undergraduate nursing students with real-time, hands-on learning experiences involving population health efforts in Frederick County. It will also formalize a partnership with the local health department to prioritize work on Maryland’s Achieving Healthcare Efficiency through Accountable Design model and related population health action plan over the next 10 years.
